Home Care can provide care during a patient's recovery, following discharge from the hospital. Home care is for patients who need skilled healthcare professionals to help them re-adjust to normal daily living.
Virtua Home Care professionals can help make the transition from hospital to home as smooth as possible. In some cases, home care professionals provide individuals with extra help to continue living at home. The home care team encourages independence by providing the support patients need to achieve the following goals:
- Recover in comfort following hospitalization
- Remain at home with family or other caregiver support
- Delay or prevent unnecessary re-hospitalizations or unnecessary emergency room visits

Home Care Services
Serving residents of Burlington, Camden and Gloucester counties since 1995, Virtua Home Care provides patients of all ages with the following services:
- Skilled nursing
Skilled nurses coordinate all of the services you need to recover; teach you and your family about care needs and disease process; monitor your vital signs and progress; update your doctor about your condition; and help with diabetes disease management, wound and surgical care, colostomy care and other procedures.
- Physical therapy
Physical therapists help individuals with health problems and/or pain which make it difficult to move and perform everyday tasks. Following an evaluation, therapists create an individualized plan of care designed to assist patients to increase their function and quality of life.
- Occupational therapy
Occupational therapists help individuals with health problems and/or pain which make it difficult to perform activities of daily living, such as bathing, dressing, or meal preparation. Following an evaluation, therapists create an individualized plan of care designed to assist patients to increase their function and quality of life.
- Speech therapy
Speech therapists help individuals with speech, language, swallowing or communication disorders. Following an evaluation, therapists create an individualized plan of care designed to assist patients to increase their function and quality of life.
- Home health aides
Home-health aides assist with grooming and personal hygiene; prepare meals; monitor changes in your condition; and help with other tasks of daily living.
Comprehensive care for chronic conditions such as diabetes, COPD, CHF: in collaboration with primary care physicians and specialists. Trained home care providers develop tailor-made care plans for these patients. Our home-based telehealth program enables your care team to receive daily reports about your health. Utilizing small devices set up in your home, the home monitoring system enables the nurse to monitor your heart rate, blood pressure, blood oxygen levels and weight on a consistent basis. This information enables your care team to work collaboratively with your physicians to track your vital signs and better manage your symptoms.
- Transitions Care Program
This program addresses the gaps in care and promotes effective communication of health –related information. Focus is to engage patients to take control of their own health.
- Nutritional support
Registered dietitians teach and support patients to maintain optimal nutrition standards for their recovery.
- Maternal Child Health Program
Services are provided by registered nurses with special skills, knowledge and experience in the care of mothers and their babies.
All team members collaborate with other healthcare providers, including physicians, to insure the best outcome.
- Private services
Private personal care services, in addition to the services you receive under Medicare or private health insurance, are available by request. These services include a nursing assessment and home-health aide services. Skilled nursing and therapy services are also available. You will be billed privately for the services you request.
- Social work
Social workers help you cope with issues related to your illness, recovery process and body image following amputation, mastectomy or other major changes; make sure you understand your rehabilitation needs; and help you find community resources.
- Billing assistance
We will help you with the billing process and, whenever possible, bill your insurance company. Our fee schedule is available upon request.
Virtua Home Care is Medicare and Medicaid certified and licensed by the State of New Jersey.
Virtua Senior Services
In addition to Virtua Home Care, Virtua Health offers the following programs and services for area seniors:
- Acute Care for the Elderly (ACE) Unit
Virtua Berlin and Memorial ACE Units prevent older patients from losing their ability to function independently. From admission, patients are encouraged to stay active and take part in the recovery process.
- Virtua VIP
This free membership program is designed for active seniors. Programs offered include breakfast of lunch seminars with guest speakers discussing ways to maintain a healthy lifestyle. Our members are also offered one day trips to local points of interest and more.
- Long-term Care
At the Virtua Health & Rehabilitation Centers in Mount Holly and Berlin, our healthcare professionals create a warm and caring environment for everyone, whether it's our long-term residents, or those recovering after a hospitalization, before returning home.
