Home Care can provide care during a patient's recovery, following discharge from the hospital. Home care is for patients who need skilled nurses to help them re-adjust to normal daily living.
Virtua Home Care professionals can help make the transition from hospital to home as smooth as possible. In some cases, home care professionals provide individuals with extra help to continue living at home and avoid entering a nursing home. The home care team encourages independence by providing the support patients need to achieve the following goals:
- Recover in comfort following hospitalization
- Remain at home with family
- Delay or prevent nursing home placement
In addition to a caring and experienced staff who provide hospital-level nursing care, Virtua Home Care is:
- Accredited by The Joint Commission, signifying achievement of the highest standards of quality care
- Recognized as one of the Top 100 home health agencies by Home Health Line and Outcome Concept Systems, and the only organization in New Jersey and one of only four in the Northeast region to make this distinguished list
- Licensed by the State of New Jersey and certified as a Medicare/Medicaid provider
