Prior to your due date, you will receive the necessary pre-registration forms. You should complete these forms and return them by mail. However, you also have the option of pre-registering for your hospital admission via this secure web site. Just complete the online e-registration form for the hospital where you plan to deliver: Virtua Memorial or Virtua Voorhees. If there are any questions, a hospital registrar will call you.
When you are ready to deliver and arrive at the hospital, a registrar will confirm the registration information with you. You will also be asked to sign a consent form upon arrival. Please remember to bring to the hospital:
- Identification (such as driver's license)
- Insurance cards
- Claim form (if necessary)
- Referral (if applicable)
- Co-pay (if applicable)
