Going home after a hospital stay is a momentous day. But recovery doesn’t end when you’re discharged from the hospital, and neither does the care from Virtua. Through our Home Care program, you can recover with your family in the comfort of your own home. Our skilled Home Care staff helps you readjust to daily life and can help prevent repeat hospitalizations or unnecessary emergency room visits.

Our caring and experienced staff provides hospital-level nursing care in your home and is accredited by The Joint Commission, signifying achievement of the highest standards of quality care. Our home care is also licensed by the State of New Jersey and certified as a Medicare/Medicaid provider.

In-home nursing
Skilled nurses coordinate services you need to recover, explain care needs to your family, monitor your vital signs and progress, and update your doctor about your condition.

Physical, occupational or speech therapy
Following an evaluation, therapists create an individualized plan for you to increase your function and quality of life.

Home health aides
Assist with daily tasks such as grooming and personal hygiene, preparing meals and monitoring changes in your condition.

Patients with chronic conditions such as diabetes, COPD and congestive heart failure can monitor their vital signs from home. Information on their heart rate, blood pressure, blood oxygen levels and weight is transmitted via telehealth technology to a nurse at Virtua Home Care for constant monitoring.

Transitions Care Program
Addresses the gaps in care and promotes communication of health-related information to help you take control of your health.

Nutritional support
Registered dietitians teach you how to maintain optimal nutrition standards for your recovery.

Maternal Child Health Program
Registered nurses with special maternal-related skills, knowledge and experience assist new moms in the care of their babies.

Social work
Helps you cope with issues related to your illness, recovery process and body image following amputation, mastectomy or other major changes. Makes sure you understand your rehabilitation needs and helps you find community resources.

Senior services include Virtua VIP and long-term care.
