Home Care

A-Z Index

  • Introduction

Home Care

Home Care can provide care during a patient's recovery, following discharge from the hospital. Home care is for patients who need skilled nurses to help them re-adjust to normal daily living.

Virtua Home Care professionals can help make the transition from hospital to home as smooth as possible. In some cases, home care professionals provide individuals with extra help to continue living at home and avoid entering a nursing home. The home care team encourages independence by providing the support patients need to achieve the following goals:

  • Recover in comfort following hospitalization
  • Remain at home with family
  • Delay or prevent nursing home placement

In addition to a caring and experienced staff who provide hospital-level nursing care, Virtua Home Care is:

  • Accredited by The Joint Commission, signifying achievement of the highest standards of quality care
  • Recognized as one of the Top 100 home health agencies by Home Health Line and Outcome Concept Systems, and the only organization in New Jersey and one of only four in the Northeast region to make this distinguished list
  • Licensed by the State of New Jersey and certified as a Medicare/Medicaid provider

Skilled nursing

In addition to caring for you, the nurse will also counsel and teach you and your family about proper health care in the home.

Home health aideS

Certified home health aides work with you and other members of your health care team to help maintain a healthy and safe environment. They also assist with personal hygiene and meal preparation.

Private services

Patients or families can self-pay for home care services such as light housekeeping, helping with dressing and bathing, meal preparation, medication reminders, light exercise and more.

Medical social services

These professionals will evaluate your ability to cope with issues related to your illness and recovery process and help you understand and accept services necessary for rehabilitation. They'll assist with the utilization of health and welfare resources, when necessary, to help you resume daily individual and community living.

Virtua's clinically experienced and licensed rehabilitation therapists provide comprehensive outpatient rehabilitation services including physical therapy, sports rehabilitation, occupational therapy and speech and audiology therapy.

Nutritional support

Registered dietitians teach and support patients who require intravenous or tube nutrition.

Telehealth program

Patient vital signs are monitored at home using special medical equipment that allows information to be transferred via wireless technology to the health care specialist.
