Virtua Heart Failure Program - Moorestown
The Virtua Heart Failure Program in Moorestown is a community-based program with offices located throughout South Jersey. The team’s proven education and treatment protocols are designed to help patients manage their congestive heart failure (CHF) and avoid unnecessary hospital visits.
Patients actively work with their physician, nurse practitioners, nurses and family members to manage their disease through medication, diet changes and appropriate activity. Through this team approach, we enhance the well-being of our patients, promote optimal heart function and reduce CHF symptoms.

- Nutrition counseling
- Education
- Remote and in-office monitoring programs, including CardioMems, CareLink/CorVue and LifeVest
- Acute care for congestive heart failure
- Intravenous care (IV Lasix)
- Medication reconciliation
- Assistance with obtaining resources including home care, physical/occupational therapy, palliative/hospice programs, home oxygen, cardiology-specific psychiatric counseling, and home infusion therapies
- Access to advanced treatments, including a left ventricular assist device (LVAD) and heart transplant
- Access to clinical trials
