  • Have a Body Mass Index (BMI) of 40 or greater, or be more than 100 pounds overweight
  • Have a BMI between 30 to 35 and have at least one or more obesity-related co-morbidities
    (such as type II diabetes, hypertension, sleep apnea and other respiratory-related disorders, non-alcoholic fatty liver disease, lipid abnormalities, gastrointestinal disorders, or heart disease)
