Proxy Access

If you are the parent/legal guardian of a child under the age of 18 and would like Proxy access to that child's health information through your MyChart account, download and sign the Parent/Child Proxy Form (English) or (Spanish) and return it to Virtua.

You can send the form in two ways:

  1. Fax the form to 856-762-2843
  2. Scan and email the form to: MyChartPHR@virtua.org

To request Proxy access to a child directly through your own MyChart account, log into your MyChart account and follow these steps:

  1. Select Menu then scroll down and select Minor Proxy Access Request Form.
  2. Fill out the form and click the Submit Request button.

I Need a Sign-Up Code

A sign-up code is needed to enroll. Open the sign-up code request form by clicking the button below. Sign the completed form and return it to Virtua. You can send the form in two ways:
 
1. Fax the form to: 856-762-2843
2. Scan and email the form to: MyChartPHR@virtua.org

 



I Have My Sign-Up Code

Enter your activation code and some personal information to enroll. Please be sure to read the MyChart Terms of Use. An email address is required as part of the enrollment process.

New Information Automatically Available Through MyChart

Beginning February 23, 2021, Virtua proactively made more of your medical information, including most new provider notes, automatically available to you in MyChart. While Virtua has tried to make your most important medical information automatically available in MyChart, MyChart does not contain your full medical record. This is because creating or logging into a MyChart account is not a request from you to Virtua for your medical records. Requesting copies of your medical records not automatically available in MyChart is simple. See the instructions in our FAQ section under “How can I request copies of my medical records not automatically available in MyChart?”
