Virtua Health is unable to grant requests for proxy access made by anyone who is not a parent or legal guardian, even if they are a family member. Please ask the parent or legal guardian to make a request authorizing proxy access.
Who is authorized to request Proxy Access for an adult?
For an adult, the process for proxy access is different than that of a child. If you would like to grant another adult access to your MyChart account, you must fill out the Adult Proxy Access Request and Authorization Form, which is available from the drop down Menu under “Sharing” in your MyChart account. The form authorizes another adult to receive access to your Virtua Health (Virtua) MyChart medical record. Your proxy can be a spouse, partner, adult child, or any other trusted adult designated by you.
